施設名、地域、お名前、電話番号、メールアドレス、お問い合わせ内容
Fitness Center Name, Location, Name, Phone Number, Email Address, Inquiry Details.

個人情報の保管期間Retention Period of Personal Information
お問い合わせ日から3年間保管し、その後適切に廃棄いたします。
Personal information will be retained for three years from the date of the corporate inquiry application and then destroyed.
お問い合わせへの対応のために必要な同意となりますが、個人情報の取得および利用への同意は任意です。
